The following attachment is for NON-LBHS guests. This form must be completed and returned no later than Wednesday, April 10th. Forms must be submitted to Ms. Levy in 7-216 or to Ms. McIntyre in the B6 office. Be sure to include the photo ID with the permission form.  Forms can be faxed directly to Ms. McIntyre 407-746-3687.

Reminders: guests must be in High School or no older than 20 years of age.
